Tinnitus Relief: What Actually Helps

The questions people ask most about tinnitus relief, each answered against the ENT clinical guideline: what is recommended, what is advised against and what is unproven.

In short

Tinnitus usually becomes less intrusive over time as the brain habituates. The AAO-HNS guideline recommends education, a hearing aid evaluation if you have hearing loss, and cognitive behavioral therapy; sound therapy is an option. It advises against supplements such as ginkgo, melatonin and zinc, and against routine medicines for tinnitus. Get prompt care for sudden hearing loss or one-sided or pulsing tinnitus.

Tinnitus searches are full of promises: pills, apps, frequencies and "cures." The most useful thing you can know is what the medical guideline actually recommends, what it advises against and what is still unproven. This guide answers the questions people ask most, each one graded against the American Academy of Otolaryngology–Head and Neck Surgery Foundation's clinical practice guideline on tinnitus.

Will my tinnitus ever go away?

It often becomes much less noticeable. Surveys suggest that tinnitus tends to decline over a period of years for a large proportion of people, and that the annoyance usually fades too, even without treatment. In some people it disappears completely.

The key idea is habituation: your brain gradually learns to treat the sound as unimportant, so you stop noticing it most of the time. Many of the approaches below work by speeding up habituation rather than silencing the sound.

What does the medical guideline recommend?

The guideline sorts treatments by strength of evidence.

ApproachGuideline position
Education and reassurance about tinnitus and management optionsRecommended
Hearing aid evaluation for bothersome tinnitus with documented hearing lossRecommended
Cognitive behavioral therapy (CBT)Recommended
Sound therapyAn option
Supplements (ginkgo biloba, melatonin, zinc and others)Advised against
Routine medicines for the tinnitus itself (antidepressants, anticonvulsants, anti-anxiety drugs, drugs injected into the ear)Advised against
Transcranial magnetic stimulationAdvised against routinely
AcupunctureNo recommendation

The guideline defines "persistent" tinnitus as lasting six months or longer, and "bothersome" as distressing enough to affect quality of life.

Does CBT actually help?

Yes, for how much tinnitus bothers you. CBT does not usually make the sound quieter; it changes how you respond to it, reducing distress, worry and the sense of being trapped by the noise. That is why it is one of the guideline's strongest recommendations. It is available from psychologists and some audiologists, and in structured app or online programs.

Will hearing aids help my tinnitus?

If you have hearing loss, often yes. Making outside sounds easier to hear gives your brain more to listen to, so the tinnitus stands out less. That is why the guideline recommends a hearing aid evaluation for bothersome tinnitus with documented hearing loss. If your hearing is normal, hearing aids are not the tool.

What is sound therapy?

Using low-level background sound, such as a fan, soft music, nature sounds or a sound generator, to reduce the contrast between your tinnitus and silence. The aim is not to drown the tinnitus out with loud sound but to make it blend in. The guideline lists it as an option.

Tinnitus retraining therapy (TRT) combines sound therapy with structured counseling. Reviews suggest it may give moderate improvement in tinnitus distress for adults with moderate to severe tinnitus; like other approaches, it does not eliminate the sound.

Do supplements like Lipo-Flavonoid or ginkgo work?

The guideline advises clinicians not to recommend ginkgo biloba, melatonin, zinc or other dietary supplements for tinnitus. Products marketed as cures deserve skepticism. See tinnitus supplements: what the evidence says.

What about Lenire?

Lenire is a device that pairs sound through headphones with mild electrical stimulation of the tongue. It received FDA De Novo authorization in March 2023 and is offered through specialist clinics in the US and several other countries. Clinics report high rates of improvement, but those figures come mainly from the company and treating clinics, providers state that it does not cure tinnitus, and it is usually paid out of pocket. It was authorized after the 2014 guideline, so the guideline does not address it.

How do I get through a tinnitus spike?

  • Add gentle background sound rather than sitting in silence.
  • Slow your breathing and do something absorbing; stress can make tinnitus feel louder.
  • Avoid loud noise and protect your ears in loud places.
  • Keep a simple log of sleep, stress, caffeine and noise exposure to spot your own triggers.

Most spikes settle back to your usual level.

How can I sleep with tinnitus?

Tinnitus often seems loudest at night because the room is quiet. A fan or soft sound set just below the level of your tinnitus can help. How to sleep with tinnitus has a full bedtime routine, and white vs pink vs brown noise explains the options.

If tinnitus is affecting your mood

Tinnitus can wear down sleep and mood, and that is a real part of the condition. Talk to your doctor about how it affects you.
